Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a free and non-commercial open data platform and infrastructure that supports research on pragmatic trials. It collects and distributes cleaned trial data, ratings, and evaluations using PRECIS-2. This permits a variety of meta-epidemiological studies to examine the effect of treatment across trials with different levels of pragmatism.
Background
Pragmatic trials provide evidence from the real world that can be used to make clinical decisions. However, the usage of the term "pragmatic" is not consistent and its definition and evaluation requires further clarification. The purpose of pragmatic trials is to inform policy and clinical practice decisions, rather than confirm a physiological or clinical hypothesis. A pragmatic trial should aim to be as close as it is to actual clinical practices which include the recruitment of participants, setting, designing, delivery and execution of interventions, determining and analysis outcomes, and primary analysis. This is a significant difference between explanatory trials, as described by Schwartz & Lellouch1, which are designed to prove a hypothesis in a more thorough way.
Truly pragmatic trials should not be blind participants or clinicians. This can lead to a bias in the estimates of the effects of treatment. Pragmatic trials will also recruit patients from various healthcare settings to ensure that the results can be applied to the real world.
Additionally, pragmatic trials should focus on outcomes that are important to patients, like quality of life or functional recovery. This is particularly important in trials that require surgical procedures that are invasive or may have dangerous adverse consequences. Many RCTs that don't meet the criteria for pragmatism but contain features contrary to pragmatism have been published in journals of varying types and incorrectly labeled pragmatic. This can lead to false claims of pragmaticity and the use of the term must be standardized. The creation of a PRECIS-2 tool that can provide an objective and standardized evaluation of pragmatic aspects is a good start.
Methods
In a practical study the aim is to inform clinical or policy decisions by demonstrating how an intervention could be integrated into routine treatment in real-world settings. Explanatory trials test hypotheses about the causal-effect relationship in idealized environments. Consequently, pragmatic trials may be less reliable than explanatory trials and might be more susceptible to bias in their design, conduct and analysis.
